Travel Vaccinations

If you require any vaccinations relating to foreign travel, you will need to submit a travel form 8 weeks prior to departure.  A link to the form can be found below or if you would prefer, a printed copy can be collected from Reception.

Travel risk & vaccine form

The form will help our Practice Nurse advise what vaccines may be required for your travel.  Once our Practice Nurse has reviewed the form, you will be contacted by one of our team to make an appointment.  If there are no available appointments or it is less than six weeks to departure you will need to go through a travel clinic.

The following travel vaccinations are free to patients and are provided by the surgery:

  • Diphtheria/Tetanus
  • Hepatitis A
  • Polio
  • Tetanus
  • Typhoid

We DO NOT provide the following travel vaccinations, you will need to contact a travel clinic for these vaccines and they are not free of charge.

  • Yellow Fever
  • Japanese B encephalitis
  • Tick borne encephalitis
  • Rabies
  • Hepatitis B
  • Meningitis ACWY

Please Note – a private prescription is required for Malaria Prophylaxis tablets and this will incur a charge.  However, should this be the case then you will be made aware of any charges before an order is placed.
